Treatment Modalities at Spinalogy Clinic

Short wave Diathermy

Short waved diathermy is the therapeutic elevation of temperature in the tissue by means of an oscillating electric current. The tissue and body organs are deepheated, which increases local metabolism, supports absorption of chronic inflammatory, post-injury and post-operation infiltrates, improves tissue elasticity and has analgesic effects.
Indication: chronic diseases of joints, tissue and sinews, chronic inflammations, both gyneacological and urological, post-operation states, urge incontinence
Contraindication: Pacemakers, direct application to metal implants, malignant tumors, acute inflammations, bleeding states, thrombophlebitis etc. During the menstrual period the procedure cannot be applied to patient´s stomach, as well as to growth zones in adolescents.

Basically IFT (Interferential Therapy) utilizes physiological effects of low frequency (<250pps) electrical stimulation of nerves. It is not associated with painful and somewhat unpleasant side effects but sometimes associated with low frequency stimulation.
Two medium frequency currents are passed through tissues in interferential therapy. Both of them are of medium frequency. When they are set up, their Path cross each other & they interfere with each other, So it is called Interference Current Therapy.

Dry Needling
Dry Needling is a procedure in which a needle, often an acupuncture needle, is inserted into the skin and muscle directly at a myofascial trigger point. A myofascial trigger point consists of multiple contraction knots, which are related to the production and maintenance of the pain cycle. How Does Dry Needling Improve Function?
  • Dry Needling of muscular trigger points causes a reflexive relaxation of the involved muscle.
  • Tiny injuries created by the needle insertion cause a local healing response in the dysfunctional, painful tissue, which restores normal function through the natural healing process.
  • Dry Needling stimulates nerve pathways which block pain by disrupting pain messages being sent to the brain.
  • Dry Needling causes a local chemical response through the release of substances which block the transmission of pain messages.
What Conditions May Benefit From Dry Needling? Dry Needling in combination with other physical therapy procedures has been shown to be of benefit with the following types of injuries:
  • Acute and chronic tendonitis

Hot Packs
A hot pack is a pad used for warming of parts of the body in order to manage pain. Localized application of heat causes the blood vessels in that area to dilate, enhancing perfusion to the targeted tissue. Types of heating pads include electrical, chemical and hot water bottles. A moist hot pack is used dry on the user's skin. These pads register temperatures from 170 to 180 degrees Fahrenheit (76 to 82 °C) and are intended for deep tissue treatment. Moist heating pads are used mainly by physical therapists but can be found for home use. They are very effective in treatment of muscle spasm associated pain.
Tens
Transcutaneous electrical nerve stimulation (TENS) is the use of electric current produced by a device to stimulate the nerves for therapeutic purposes. The unit is usually connected to the skin using two or more electrodes. A typical battery-operated TENS unit is able to modulate pulse width, frequency and intensity. Generally TENS is applied at high frequency (>50 Hz) with an intensity below motor contraction (sensory intensity) or low frequency (<10 Hz) with an intensity that produces motor contraction. TENS is a non-invasive, safe nerve stimulation intended to reduce pain, both acute and chronic.
Ultrasound
Therapeutic ultrasound in physical therapy is alternating compression and rarefaction of sound waves with a frequency of >20,000 cycles/second. Therapeutic ultrasound frequency used is 0.7 to 3.3 MHz. Maximum energy absorption in soft tissue is 2 to 5 cm. Intensity decreases as the waves penetrate deeper. They are absorbed primarily by connective tissue: ligaments, tendons, and fascia (and also by scar tissue). Therapeutic ultrasound may have two types of benefit: Thermal effects and non thermal effects. Thermal effects are due to the absorption of the sound waves. Non thermal effects are from cavitation, microstreaming and acoustic streaming. Cavitational effects result from the vibration of the tissue causing microscopic air bubbles to form, which transmit the vibrations in a way that directly stimulates cell membranes. This physical stimulation appears to enhance the cell-repair effects of the inflammatory response. Therapeutic ultrasound is sometimes recommended for muscle as well as joint pain.
